*Serious incident: an incident that either led to or has the potential to lead to the significant health or public health outcomes including death or the temporary or permanent serious deterioration in the state of health of a patient, user or other person.
**Serious public health threat: a serious incident that may require prompt remedial action, and that may cause significant morbidity or mortality in humans, or that is unusual or unexpected for the given place and time.
***Serious adverse drug reaction: a noxious and unintended response to a drug that occurs at any dose and that: requires in-patient hospitalization or prolongation of existing hospitalization; or causes congenital malformation; or results in persistent or significant disability or incapacity; or is life-threatening or results in death.
